Employee Education Program Application

Use this web form to create your EEP application. Once the information is entered you will print your application and obtain all required signatures. Under the EEP, eligible employees may take up to six credit hours of college-level courses tuition free each semester.

Follow the simple steps below to create your application form:

  1. Print the detailed instructions
  2. Enter name and UFID in box below
  3. Complete all the required fields listed and confirm your personal information
  4. **Print** two copies of the form (maintain one copy for your records)
  5. Obtain required signatures, register for classes, and return to the office listed below by the EEP application deadline

**EEP will not pay for classes registered outside of these established timeframes.

**PPD-Physical Plant employees should still turn in a copy of their EEP application to the PPD Human Resources office.
