How to Enroll
Choose one of the following ways to enroll:
- Enroll online via the People First (PF) web site.
- Call PF at 1-866-663-4735 to enroll or make a change by phone.
- Mail your form or fax it to PF at 904-828-6092.
- Attend the UF Benefits’ Group Enrollment Session for assistance with completing your online enrollment with People First.
Be sure to retain a record of your transactions. If an early effective is requested for the basic life plan, please contact the benefits office at 392-2477 regarding payment of premiums.

Coverage
First 60 days of employmentFixed term. Offers coverage for the employee, spouse, and child. Guarantee issue amount up to $100,000. Optional spouse and child coverage for 1/2 issue of employee; maximum allowable for child coverage is $25,000.
Coverage
Annual open enrollment$10,000 for employee; $5,000 for dependents, subject to 50 percent of employee coverage, and maximum plan limits up to $25,000 on the child.
Comments
Can apply via medical application at any time for amounts over guaranteed renewal amounts during open enrollments.
Eligible dependents:
- Children under 19 years old
- Children ages 19-23 and are full time students
Disabled children are eligible for coverage as long as they remain dependent. Dependents who no longer meet eligibility criteria must be removed from coverage by contacting the University Benefits Office.

Choose one of the following ways to enroll:
- Complete an enrollment form and return it to the Benefits office by campus mail to P.O. Box 115007; fax to 352-392-5166; or take it to the Human Resource Services satellite office nearest you.
- Attend the UF Benefits’ Group Enrollment Session for assistance with completing your UF Term Life enrollment in addition to other plans.
